Widely Used Sugar Substitutes May Be Associated with Faster Declines in Memory and Thinking Skills Over Time

A recent study published in Neurology, the esteemed medical journal of the American Academy of Neurology, has unveiled a potentially concerning association between the consumption of several popular low- and no-calorie sweeteners and a more rapid decline in cognitive functions, including memory and thinking skills, over time. The research, which meticulously tracked nearly 13,000 adults, suggests that individuals with the highest overall intake of these sweeteners experienced a more pronounced deterioration in cognitive abilities compared to those with the lowest consumption. The extensive research initiative, conducted with participants residing across Brazil, enrolled 12,772 adults with an average age of 52 years. These individuals were monitored over a substantial period of approximately eight years, allowing for the observation of longitudinal changes in cognitive function. To accurately assess sweetener consumption, participants completed detailed food questionnaires at the commencement of the study, providing a retrospective account of their dietary habits from the preceding year. This data allowed researchers to categorize participants into three distinct groups based on their total daily intake of sweeteners.

The quantitative analysis revealed a significant disparity in consumption levels. Individuals in the lowest intake group averaged a modest 20 milligrams (mg) per day, while those in the highest consumption group reported an average daily intake of 191 mg. For context, the higher end of this spectrum, specifically concerning aspartame, equates to roughly the amount found in a single can of diet soda. This comparison highlights how easily individuals can exceed levels that researchers have linked to cognitive changes, especially when considering the cumulative effect of various products.

Among the individual sweeteners analyzed, sorbitol emerged as the most frequently consumed, with an average daily intake of 64 mg among participants. This finding underscores the widespread use of sugar alcohols, often found in sugar-free gums, candies, and baked goods, as a perceived healthier alternative.

To meticulously track cognitive health, participants underwent a series of standardized cognitive assessments at three distinct intervals: the beginning of the study, the midpoint, and at its conclusion. These assessments were designed to measure various facets of brain function, encompassing verbal fluency, working memory, word recall, and processing speed.

Verbal fluency, a critical indicator of cognitive agility, refers to an individual’s capacity to rapidly retrieve and articulate words. Working memory, essential for tasks requiring immediate information retention and manipulation, represents the brain’s temporary storage and processing system. Processing speed, on the other hand, gauges the efficiency with which a person can comprehend and react to incoming information. The consistent application of these tests across the study’s timeline provided a robust dataset for evaluating changes in cognitive performance over the eight-year period.

Quantifying the Association: Higher Intake, Faster Decline

After meticulously adjusting for a range of potential confounding factors such as age, sex, the presence of high blood pressure, cardiovascular disease, and other relevant health and lifestyle variables, the researchers identified a statistically significant and concerning trend. Individuals who reported the highest consumption of sweeteners demonstrated a 62% faster decline in their overall thinking and memory abilities when compared to those in the lowest consumption group. The researchers further estimated that this accelerated cognitive decline was equivalent to approximately 1.6 additional years of cognitive aging.

Participants in the middle consumption group also exhibited a noticeable acceleration in cognitive decline, experiencing a 35% faster rate than their counterparts in the lowest intake group. This difference was estimated to be comparable to approximately 1.3 years of accelerated aging. These figures provide a tangible perspective on the potential long-term impact of high sweetener consumption on cognitive longevity.

Age and Diabetes: Key Modulating Factors

The study’s findings also revealed interesting nuances regarding the influence of age and pre-existing health conditions. The association between higher sweetener intake and accelerated cognitive decline was particularly evident among participants younger than 60 years old. In this demographic, those with the highest sweetener consumption exhibited faster declines in verbal fluency and overall cognitive performance compared to their lower-consuming peers. Notably, this association was not observed in participants aged 60 and above. This suggests that younger individuals may be more susceptible to the potential negative effects of these sweeteners on brain health, or that the cumulative effects of other age-related cognitive changes might mask the impact in older adults.

Furthermore, the link between sweetener intake and cognitive decline was significantly stronger in individuals with diabetes. This finding is particularly noteworthy, as people with diabetes are frequently advised to limit products that cause rapid spikes in blood sugar, leading many to adopt sugar substitutes. The heightened association in this group raises questions about whether the metabolic conditions associated with diabetes might amplify the impact of sweeteners on the brain, or if individuals with diabetes might be consuming a wider array or higher quantities of these products due to dietary recommendations.

The research delved into the individual contributions of the seven sweeteners examined: aspartame, saccharin, acesulfame K, erythritol, xylitol, sorbitol, and tagatose. When analyzed separately, six of these sweeteners were found to be associated with faster declines in overall cognition, with a particular impact on memory. These six included aspartame, saccharin, acesulfame K, erythritol, sorbitol, and xylitol. Tagatose was the sole sweetener in the study that did not demonstrate a statistically significant link to cognitive decline.

This granular analysis is crucial for informing consumers and manufacturers alike. The widespread use of these sweeteners across a vast array of food and beverage products means that even individuals who consciously avoid one particular sweetener may still be exposed to others that carry similar risks.

Dr. Claudia Kimie Suemoto, MD, PhD, a lead author of the study from the University of São Paulo in Brazil, offered insights into the findings: "Low- and no-calorie sweeteners are often seen as a healthy alternative to sugar; however, our findings suggest certain sweeteners may have negative effects on brain health over time." Dr. Suemoto further elaborated, "While we found links to cognitive decline for middle-aged people both with and without diabetes, people with diabetes are more likely to use artificial sweeteners as sugar substitutes. More research is needed to confirm our findings and to investigate if other refined sugar alternatives, such as applesauce, honey, maple syrup, or coconut sugar, may be effective alternatives."

The research did not encompass every artificial sweetener currently available on the market, meaning the findings cannot be universally applied to all sugar substitutes. Consumers may be exposed to a wider array of sweeteners not included in this specific analysis.

Furthermore, the dietary information was self-reported by participants. While researchers employed rigorous methods to gather this data, the accuracy of self-reported dietary intake can be subject to recall bias and estimation errors. Individuals may inadvertently forget certain food items or misjudge the quantities consumed.

Most importantly, as an observational study, it identified a correlation between higher sweetener consumption and a faster rate of cognitive decline. However, it cannot definitively prove that the sweeteners were the direct cause of these cognitive changes. It is plausible that individuals who consume more sweeteners might also engage in other lifestyle behaviors or have underlying health conditions that contribute to cognitive decline, even if these factors were adjusted for in the analysis.
